What Is Hospice Care?

The word ‘Hospice’ means to care for the sick or terminally ill. The word ‘Care’ is the provision of what is necessary for the health, maintenance, and protection of someone. In essence, hospice care is the application of respect for a person’s comfort, dignity, and privacy during a time of great need and vulnerability.  Hospice care with R2R Healthcare gives the individual the assurance of the best possible quality of life through guidance with symptom and pain management, as well as education and emotional support for the patient and their loved ones during this crucial time. Our team of medical professionals will work together as an interdisciplinary team with patients and family members to develop an individualized care plan. This plan will be specifically tailored to the patient’s goals related to the physical, emotional, and spiritual aspects of managing their terminal illness.

Medicare and Hospice

100% Coverage if These Four Requirements Are Met:

  • The patient is eligible for Medicare Part A (Hospital Insurance).
  • The patient’s attending physician (if any) and a hospice physician must certify that the patient is terminally ill and has 6 months or less to live if the illness runs its normal course.
  • The patient signs a statement choosing hospice care instead of other Medicare-covered benefits to treat the terminal illness.
  • The patient receives care from a Medicare-approved hospice program.

Diagnoses

Hospice is often associated with cancer when, in actuality, less than half of all hospice patients have that diagnosis. A person with any advanced, terminal illness is eligible for hospice.

In addition, those experiencing serious multi-symptom deterioration, frequent emergency room visits and/or hospitalizations, or those discontinuing aggressive treatment may also be eligible for hospice services.

  • Grief counseling for patient and family
  • Short term in-patient and respite
  • Medications for symptom control or pain relief
  • Medical supplies and equipment related to the terminal diagnosis
  • Hospice aide and homemaker services
  • Physical and occupational therapy, as appropriate for the plan of care
  • Speech-language pathology services, as appropriate for the plan of care
  • Any other Medicare-covered services needed to manage pain and other symptoms, as recommended by the hospice team

How to Begin Care with R2R Palliative and Hospice Care

Anyone can refer a patient to R2R; patients can even refer themselves. Eligibility for hospice services must be certified by the patient’s attending physician and the hospice medical director. If you need help with a referral, contact R2R at 972-219-0020. We will guide you through the process.
